CVS Brent VACANCY – Senior Hub Co-ordinator – £31,000 two year fixed term post, with extension subject to available funding.
The Social Isolation in Brent Initiative (SIBI) project seeks to build on a model designed with the input of key stakeholders and implement a scheme aimed at tackling social isolation within the community. The potential SIBI client group is all individuals over the age of 18 who are at risk of social isolation; this wide client group aims to tackle social isolation in all of its forms. The objective of the SIBI project is to target and address social isolation through assisting individuals identified as being at risk of social isolation, lack of social contact and low community involvement. The aim of the project is not to duplicate existing services but to complement existing services. The service will also support the development and utilisation of novel services to address social isolation in the LB Brent. The service is co-sponsored by LB Brent and Brent CCG. The SIBI project will have a static location, which will run 5 days/week and a mobile hub, which will run in various locations and days of the week depending on the demands of the service. The service will be staffed by a full time Co-ordinator who will develop, plan and run both the static and Mobile Hub services with the assistance of volunteers and input from a range of healthcare and other statutory services on the Mobile Hub. The post holder will provide generic information, advice and signposting to service users as well as the set up, planning, organisation and day to day management of both the static hub and outreach services including the mobile hub. She/he will have responsibility for the recruitment, management and training of volunteers based in SIBI service. Additionally they will have responsibility for the implementation, monitoring and continuing development of services to address social isolation in LB Brent. The post holder will also work in partnership with NHS Brent CCG and act as the main point of contact for Multi-Disciplinary Group (MDG) members and voluntary and community organisations linking in to SIBI services.
